Who is the service for?
Anyone living in the Cities of Brimbank, Maribyrnong, Hobsons Bay and Wyndham who has reached a stage in their illness when palliative rather curative treatment is considered appropriate are eligible.
Who can make Referrals?
Referrals are welcome from self, doctor, relative, friend, or hospital.
