Does the Assessment Team replace my doctor?
No. Your doctor will still be the main person for your ongoing medical care. However if you are having difficulty managing at home or are thinking about hostel or nursing home care, your doctor may refer you to an Aged Care Assessment Team, just as he/she would refer you to a specialist. Assessment Teams work co-operatively with the doctors in their region. They will let your doctor know what care services they have advised for you.
Your doctor will still be the main person responsible for your ongoing medical care.
