ACAT

What is an assessment?

An assessment is a consultation with one or more members of your local Aged Care Assessment Team.

A member of the Team may visit you in your home and discuss your health, and social and personal needs. with your approval, your doctor may let the Team know about your medical history

What are my rights?

  • You have the right to be treated with dignity and respect
  • You have the right to information...
  • Confidentiality...
  • Your own views and ideas...
  • Some-one of your choice with you...
  • An interpreter if necessary
  • An independent advocate...
    Contact your nearest office of the Dpt of Human Services and Health for more information on advocacy.

Where will the assessment take place?

Most Assessment Teams will make you an appointment to see you where you live.

Will the assessment cost me anything?

No. Aged Care Assessment Teams are government funded and you will not be charged for visits by Team members.

The aim is to help you chose the available services that most suit your needs.
