Please fill in all details on the form.
If you wish just to join the club you only need to fill in the top price section of the form.
If you wish to join the club and the ACTA you will need to fill in the bottom price section of the form.
You my send this form in the mail with a cheque or attend the club on operating hours.
Operating hours Tuesday,Saturday and Sunday between 12 noon to 5 pm.
