Change of Name

An application to change a name can be made by any person aged 18 years or over who was born in Victoria or is ordinarily resident in Victoria.

Birth Certificate:

Your existing birth certificate will be cancelled and a new birth certificate will be issued in the new name. The new birth certificate will retain your originally recorded sex at birth and it is standard policy in Victoria now to include the Change of Name and the Birth Certificate records on the one sheet. Your previous name will be printed in full detail on the reverse side of the Birth Certificate.
