MEMBERSHIP APPLICATION INSTRUCTIONS

Post completed form to the Treasurer, Life Activities Club Whitehorse (Inc), P O Box 245, Nunawading 3131 with a cheque/money order payable to Life Activities Club Whitehorse (Inc) for your membership fee(s)
For 2008-2009 the following per person pro-rata membership fees system applies:
(a) Applications received October to April - $18.00 plus a $5.00 joining fee
(b) Applications received May to July - $10.00 plus a $5.00 joining fee
(c) Applications received August to September - $18.00 plus a $5.00 joining fee which entitle the new member(s) to membership until September in the following year.
Receipt of Your Membership Card(s) is confirmation of acceptance into the Club. It should be carried when attending any Club activities.
Membership subscription covers the cost of running the Club (Newsletters, postage, membership card, insurances) and entitles you to join any of the activity groups organised by the Club (subject to vacancy) as well as attendance at Members meetings. Activity groups set their own charges.
