If you would like to download & print your own form (two pages), please forward your cheque or money order to our post office box as indicated on the form, you can download it from here: MEMBERSHIP  FORM (right click: Save As - to your PC)

The other option is to either bring this form with payment & see our Committee on Tuesday / Thursday nights or collect & complete the forms provided at the Club.
