A reasonable temperature is around 60deg Celsius. However there is a high salt content in a dishwasher which has an effect too.
I would suggest sterilising with well water as it will not have been chlorinated.
We use chemical sterilisation with Milton (rememebr I am in the UK) or how about a steam steriliser - they only take a few minutes and cost around $40 I guess.
I have a degree in Microbiolgy so I perhaps tend to be more thorough, but it would appear that people who do not routinely sterilise have few problems, but in the UK all Health Professionals advice sterilising all bottles before each use as a matter of routine.
