To get your research started and to avoid duplication
of information which may result in extra expense for you, we
ask that you please print off, complete and post the following forms -

With your permission, the pedigree chart information will be entered into a database for further research verification.
Use the following guidelines to assist you in completing the pedigree chart.
your FATHER is number 2;
your MOTHER is number 3;
your FATHERS PARENTS are numbers 4 & 5;
your MOTHERS PARENTS are 6 & 7 and so on..
A starting fee of AU$25.00 is required with all research requests. You will be notified of any additional charges.
To enable contact by the researcher please provide your postal address / street address as a backup to e-mail addresses as over time these my change.
